The social worker is responsible for the initiation, development and completion of a full range of psychosocial services as part of a multi-disciplinary treatment team. The social worker assesses current psychosocial functioning and the level of functioning necessary to effectively manage pre- and post-transplant medical, social, financial, and psychological demands. The social worker assesses adequacy of family and other support network to support demands of transplant and provides independent professional assessment to transplant team regarding advisability of transplant. The social worker is responsible, with the transplant team, for education of the patient and family. The social worker provides leadership to other members of the team and guidance to the patient and family regarding community resources and pharmacy coverage for transplant medications. The social worker works with the transplant team toward the goal of safe and expedient discharges. The social worker will be the liaison and coordinates interactions with patients, families, community agencies and the hospital treatment team. The social worker will carry a complicated, high-risk caseload requiring a high level of experience, sophistication, and skills.
PA LSW Experience with transplant patients preferred. Strong problem-solving skills required. Must have demonstrated ability to work within a team and to work toward team goals. Must also have demonstrated ability to make independent, professional assessment and present a dissenting view within a team. Must have demonstrated ability to be self-directed and resourceful with ability to manage complex patient issues and referrals. Demonstrated knowledge of or experience with transplant patients preferred.
