Job Description

Completes assessments to assist in identifying each participant’s needs and desires for service delivery. Assists the participant in the development of their Individual Support Plan (ISP) using a person-centered approach. The ISP should meet the participant’s identified needs in the least restrictive manner possible. Updates the ISP annually and as needed throughout the service year.

Ensure the participant choice of providers by providing information for the participant to make fully informed decisions. Identifies services and supports that will meet the participants needs and desires and completes necessary referrals. Resolve any barriers to the delivery of service. Monitors the services monthly and/or quarterly to ensure they are being delivered as planned and that outcomes and goals are being achieved and the participant is safe and in good health. Assists in identifying and sustaining the participants’ natural supports including family, friends and associates. Develops and maintains a professional working relationship with the participants and their families/caregivers. Provide required units of service per week.

Writes complete and accurate progress notes daily per individual served. Submits required documentation on time per company policy (which may be subject to change). Fulfills expected work load as assigned. Performs other duties as assigned.

BSW with a minimum 3 years related experience providing case management for individuals in healthcare. Use of reliable personal transportation. Valid Pennsylvania driver’s license including personal vehicle insurance coverage for liability and current vehicle registration. Must maintain proficiency in company sponsored training and certifications specified and/or required by future company regulations. Clean criminal background history. Successfully complete and maintain training courses as required or amended by the Office of Long Term Living for Service Coordination.

Must have experience in HCSIS or SAMS Harmony database system with Medicaid Waivers – Efficient in Microsoft Excel.

Company based in Philadelphia, but covers, Philadelphia, Delaware, Bucks and Montgomery Counties (mileage reimbursed). Monday through Friday, 9-5pm, 37.5 hours per week. Two days are in the field, three days in the office.
