The Service Coordinator I (SCI) facilitates community-based long-term care to disabled and/or chronically impaired older adult participants. This position carries responsibility for ongoing care planning, service arrangement, follow-up, and reassessment. The Service Coordinator I is expected to provide a designated number of units of billable service coordination activity (Aging Waiver) on a daily basis, including data entry and participant contact. The SCI is a participant advocate as well as the gatekeeper of service provision.
BSW, MSW with at least 12 college-level credit hours in sociology, social welfare, psychology or gerontology. One-year experience in social work. Must have a valid driver’s license, good driving record, and continuous access to a fully insured car.
