Monitors medical, educational, social, emotional and psychological needs of each foster child on the caseload. Ensures that all agencies and individuals involved with the foster child (including foster parents, schools, community agencies, courts, DHS, physicians and legal family members) are working together as a team. Coordinates educational placement, concerns, linkage and maintenance to all necessary external community resources. Maintain all necessary documentation including service plans and quarterly reports according to contract guidelines. Provide bi-monthly safety checks within the home, providing skill building and consultation to foster family and child.
