Job Description

This position is responsible for completion of initial Needs Assessment Tool to obtain Needs Assessment Score and to create waiting care plan for placement on OPTIONS Wait List. Position is also responsible for care management of individuals on the OPTIONS Wait List and individuals who are receiving home-delivered meals while waiting for case assignment for other services. This position conducts semi-annual home visits and annual reassessments to determine any changes to consumers’ health and social status, provides information about other community resources, and updates Oracle and SAMS data bases. Position may provide regular telephone contacts as needed.

BSW including at least 12 college credit hours in social welfare psychology, sociology or gerontology. Minimum of one-year social work or care management experience in the field of aging, behavioral health or disabilities. Must have valid driver’s license, good driving record and continuous access to fully insured car. Drug testing required.
